Please complete this renewal form if your charity/organization received funding for your program within the past two years, in which case this form is intended to streamline your renewal by replacing the full Program Proposal Application.

Our submission deadlines are the 15th of January, April, September, and November, and we reply within a few weeks thereafter. Please submit the completed form to your MGF contact or to office@marysgivingfund.org . If you have any questions or need help, please reach out via email to office@marysgivingfund.org.
