Mary's Giving Fund Renewal Application Form

Please complete this renewal form if your organization received funding for this program within the past two years. This form replaces the full Program Proposal Application and is designed to streamline renewals for programs with a proven record of impact.

Our submission deadlines are the 15th of January, April, September, and November. Please submit the completed form to your MGF contact or to office@marysgivingfund.org .
